Physical Therapy appointments are clinical services directed towards improving your physical health and quality of life. If you have pain, difficulty with movement, and/or physical limitations from an injury or surgery, then a Physical Therapy Evaluation can help you get you back on track.
Weightlifting Analysis and Performance and Wellness Coaching appointments focus on improving the quality of your movements so that you can prevent injuries before they happen. If you are looking to optimize your abilities with efficient movement patterns that facilitate better performance and body mechanics, then these appointments are for you.
If your primary focus is weightlifting, choose a Weightlifting Analysis Assessment. For other athletes or those who want to prevent injury generally, book a Performance and Wellness Coaching Assessment.

No, a referral is not needed to be seen at Equilibrium Physical Therapy. If you plan on submitting a claim to your insurance, note that some insurers do require a physician referral for reimbursement. Please contact your insurance company to find out specifics on out-of-network physical therapy before visiting if you plan on filing for reimbursement. If you already have a referral and/or protocol from your doctor, be sure to bring it with you to your appointment.

Equilibrium Physical Therapy is In-network with all Regence and Cigna plans and is currently adding more to our network. Call me for the latest information. Even if your plan is Out-of-network, you are likely still eligible for reimbursement from your insurance company.

Yes, clients seen for physical therapy services can submit claims to their insurance. Most insurance companies provide online claim submission via your website log-in. Be sure to submit your request with the necessary receipts and treatment codes that will be provided upon request at the clinic. The amount of reimbursement or payment toward your deductible is completely dependent on your insurance plan. If you are planning to submit for reimbursement I suggest calling your insurance company prior to your visit. Ask about reimbursement for out-of-network physical therapy expenses, and make sure there are no other requirements like a pre-authorization before you get started. For wellness services, such as Weightlifting Analysis and Performance and Wellness Coaching, insurances will consider this a preventative service and not provide reimbursement.

Yes. I accept cash, checks, or cards (debit or credit), including Health Savings Account (HSA) and Flexible Spending Account (FSA) cards. Use of FSA and HSA cards is only allowed for medically necessary services, such as physical therapy visits.

Please have completed all paperwork emailed to you and read my Notice of Privacy Practices. Try to arrive approximately 5 minutes before your scheduled visit and wear loose fitting, comfortable clothes so that I can easily assess your movement and mobility. If you have already seen a referring provider, please bring your referral and any protocols that you were provided.

Yes. If you are feeling under-the-weather or unable to participate in traditional in-person services, for whatever reason, I am happy to provide you with Telehealth+ services as an alternative to in-person care. To ensure a smooth and interruption free session, I suggest the following:
1) Turn off all other streaming services and mute calls/notifications.
2) Find a room or area of your home that you can be alone and un-interrupted.
3) Double check that your internet, camera, and microphone are operational prior to the call.
4) Position yourself in a space where you have enough room to allow for a full-body view on-camera.
